Basically all calls were either ended because voicel_mail, customer didn't answered or hangup by customer.

Here's one with the same issue.. e4ed342f-acbb-40f1-9d9e-4c1b5b1e5a6f Call went flawless actually.. then all of a sudden call ends. [ERROR] end-of-call-report Server URL Error: Request failed with status code 404

I am doing thousands and the number of failures are hundreds
the probably all have same reason

Sometimes when the calls fail in the web ui it says call ended because room was deleted..

We do 2000-4000 calls daily and we could see the Vapi webhook through Twilio was giving a 502 time out response. Our last 200 calls in the last hour have all been processed without error and their team let me know it is actively being addressed. I assume progress has been made.
