Agent repeating itself

Please help. I have seen this a few times without completion, so I wanted to bring this up as well. The agent seems to repeat the same question or response back to back without letting caller respond. I cannot duplicate an exact phrase or bug it seems random but happening a lot! I have tried in the prompt and in the Knowledge base to sort it out with no success. It really disrupts any call. I even created a differnt agent to see if that was the problem, continues to happen. Here are several examples with Agent IDs In my prompt:- Avoid Repeating Confirmations: - Only confirm once unless the user indicates confusion or requests repetition.
