I was having concurrency issues when I had make.com scenarios set to active with no filters applied to prevent it from spamming requests. I think it was rate limiting my phone numbers but I can't find any info on Vapi's rate limits so I'm not 100% sure. As soon as I made better Make.com scenarios that are only allowed to fire up to 1 request per sec, my concurrency limit issues went away
