Call transfering hangs up

Hello! I'm trying to transfer my calls under some circumstances, the agent says transferring the call now, then hangs up on me. In the logs it says "Forwarded...." What is happening, can you help me?
update: only does this with outbound calls. Inbound calls works fine.

Configure your Vapi assistant to use warm transfers instead of blind transfers, ensuring the agent stays connected while the call is forwarded, and double-check that transfer destinations are correct and reachable.
