Is anyone having issues with the query_tool not returning results on first try? My agent executes the query tool (I can see it in logs) but doesn't use the results to respond to the user. If I ask the exact same question again, then it mentions what it found from the knowledge base. Using .txt files, GPT-4o, and have explicit instructions in prompt to use query results. Anyone seen this before?
