I am having trouble working out why my Transfer tool is not working, hoping to get some help

So i imported a number from twilio and when I prompt the assistant to transfer it does one of the following 1. It says it will transfer and nothing happens 2. It says it will transfer and there is a delay and then the call ends 3.
