Since you’re using blind transfer (REFER) with Twilio, the issue is most likely that Twilio isn’t completing the transfer to the PSTN number. When that happens, the call just drops instead of being bridged. It usually needs to be handled via TwiML rather than relying fully on the SIP REFER.
