It is a twilio issue, you need to contact their support for it.

Monitor -> Logs -> Calls -> Find your Call -> Click on your call

Yeah they ended up giving me a free number. If you get the 604 error, thats what I had

yes but perhaps it works for some countries UK/US, and not others? the ones that failed were Greece and Bangladesh

You can't make calls to other country with our number.
You can only call on US numbers

Perhaps just US and UK. Is there a way to do international numbers with Twilio or do I need to use web calls?

You need to use your twilio account and enable the international calls.
